Does Health Reform Affect Long Term Nursing Care?

This question, about how health reform may affect long term care, is one that should be of pressing concern to people in middle age or seniors. After all, though the good news tells us we may expect to live longer, the bad news is that we can also expect to spend some time a nursing faciity.

Does Medicare Pay For Nursing Care?

Today, Medicare only covers short term nursing care in some circumstances. Medicare does NOT pay for long term nursing care. People must rely upon their own finances, long term care health insurance, or Medicaid for help with these bills. This includes the expense of nursing homes, assisted living, or nursing care at home.

So why doesn’t everybody buy insurance? It may be the cost of long term care insurance. But compared to the costs of not having it, the premiums may not be that much of a burden for middle income families. If you are still trying to decide what you should do, you may want to look at this older article we have posted: Pros and Cons Of Long Term Care Insurance.

Health Reform Updates

So, exactly how will health reform help people afford long term care? Apparently there are a number of clauses that refer to long term care in the bill. I have not seen much discussion of these in the news.

This is from an article on Health Leaders Media:

One of these provisions is the Community Living Assistance Services and Support (CLASS) Act, which was originally introduced by the late Senator Edward Kennedy. The CLASS Act will make long-term care insurance available to all Americans, who will be automatically enrolled with the choice to opt out.

Apprently, after a 5 year waiting period, the plan will provide a $50 a day benefit to people who are enrolled. This benefit will not be enough to cover the costs of an actual nursing facility, which may be 2 – 3 times that, but it is a start. So it would not totally stop people from considering private policies or other alternatives.

So from what I understand, a person must be enrolled in this plan for five years before they can collect. This is to keep people from enrolling the day after they have an issue I guess, but really only help those who made long term plans. That is, however, how insuance usually works.

Discuss!

I’m sure this provision will start some conversations. You can always register for free here to tell us what you think!

The Federal High Risk Health Insurance Pool May Be Coming To A State Near You!

Declined For Health Insurance

As one of the first steps towards health reform, the federal high risk health insurance pool is moving in to many states as an alternative to the current high risk health plans for people who have been declined by private insurers. In the long run, by 2014, this system should be replaced by health insurance exchanges that cannot decline because of prior health issues and cannot add rate increases because of health. This step is a transition, and should give a lot of people some relief that they need right now.

Note: Individual Health Insurance Quotes in Your State

Keep in mind, that each state has different ways of serving high risk individuals with pre-existing conditions. But judging by my own state, it looks like this may be a friendlier system with lower premiums and no waiting periods for qualified applicants. The current system in my state, for example, can impose a 12 month waiting period for pre-existing conditions under some circumstances, and has premiums that are twice the market rate that people who were accepted into private medical plans would pay.

The government has allocated $5 towards the federal high risk health insurance system. The idea behind it is to expand coverage for people with pre-existing conditions while removing some barriers like a very high cost or waiting periods. They will not do away with the state programs, at least not right away. One state website informs peope to check out both programs to see which one will serve them better right away.

How To Learn About the Federal High Risk Pool In Your State

Apparently states are all phasing in this program, and some are behind schedule. A quic search for the high risk health plan in your state should take you to a website with the information you need. You can aso visit this Department of Heath and Human Services High Risk Health Tip Sheet for more information.

You can also find contact information for the state insurance departments here.

Basic Criteria for the Federal High Risk Program

Be a US Citizen, National, or Legal Resident

Not Have creditable coverage for the previous 6 months

Have a qualified pre-existing health condition

The rate is capped at the standard rate which a regular applicant would pay for individual major medical. This is subsidized, for now, by $5 Billion dollars of federal funds.

Beware of Health Reform Scammers

Health insurance scams are not new, and in fact, there has been a reported increase even before the new health reform bill passed. A troubled economy has created a larger population of Americans who were struggling with premiums because premiums still rose while incomes fell. That is one reason why President Obama’s promise of health care reform was so attractive to many of us in the USA. This environment, high costs and low incomes, always seems to breed scammers and people who are vulnerable to falling for them.

But some confusion over the new laws, and especially, when and how they will be put in force, has generated a new way of health insurance scams. I post this here because older people may be a very vulnerable group. Middle aged people tend to struggle with higher major medical premiums, and seniors are, sadly, a favorite victim of Medicare scams. You can find some examples of scams that exploit health reform here.

How Can You Protect Yourself?

Follow the old adage that if something sounds to good to be true, it probably is.Some scammers are very slick, and I even had a lawyer friend of mine ask me to check one out because she thought it was real. She had a sales call that claimed she could replace her $1,000 a month family health insurance bill with a $99 alternative. When I called, it was really hard to pin this company down too. I asked the guy, “Is this health insurance?” His responses went from evasive to downright hostile. Any health health plan with a really low premium could really be a discount plan, or it could just not even exist. Some glossy brochures and a smooth salesperson do not make a health insurance company legitimate! But there have been cases of actual licensed agents getting duped by a slick presentation because they had not done their homework. Consumers, who are not eduated in insurance would be a much easier target!

You know you can check out any company or agent with your state insurance department. Please do that before you risk your family’s health and finances.

Check out offers or company names at online scam boards or forums. There are a lot of websites that do list scam reports. Keep in mind that some of these could just be posts from disgruntled customers of legitimate insurers, but you can still check for any red flags.

The federal government is not sending out workers to sell “Obamacare” policies. Some sales peope, and even agents, have used this tactic to get in the door at customer’s homes to sell private medical insurance or Medicare health plans.

Do not respond to pressure. If you are interested in an offer, a legitimate insurer or agent will give you time to check it out. Again, your state insurance department is a great resource!

Health Reform in The USA

Since the health care reform debate seems to keep dragging on, I figured it might be interesting to some of us if I outlined a short summary of the history of this issue. Some of you probably think this all just start popping up this year, or last year. You would be wrong. Your parents and grandparents probably debated this issue. And it looks like their generations actually got something done!

If you think I left something out, please feel free to register and comment! These are just notes, rather than an essay. I just wanted to give everybody an outline of the basics of heath reform ideas, progress, and battles in this century.

By the way, I got inspired for this by Health Care Reform – Where are We Now?

Teddy Roosevelt, FDR and Truman

In the 1910s Teddy Roosevelt was actually a progressive candidate. He campaigned on the promise of national health care, women’s voting rights, and safer working conditions for workers. He lost that campaign to Woodrow Wilson.

First Heath Insurance? In 1929, the year my own mother was born, Baylor Hospital in Dallas started a pre-paid program with a local teacher’s union. They intended to build a model for health insurance. A couple of years later, and Oklahoma doctor formed a farmer’s cooperative health association. Members formed a flat fee, and they could access health care.

The First Time Medical Plans Were Called Socialist: Wilbur was the interior secretary for the US in the 1930′s He formed a commission to study the issues of affording health care, which was found to be beyond the reach of many Americasns. This study called for the formation of group health insurance plans. Guess what. The American Medical Association called it socialist!

As early as 1935, FDR wanted a national health care plan to be included in the Social Security Legislation. Harry Truman, the next president, wanted a fund that everybody would contribute to, like we contribute to social security, to provide the money for this plan. This was left out of the New Deal, and the AMA continued to call it socialist.

Growth of Early Health Insurance:But the late 1930′s, pre-paid hospital plans are becoming more popular though. They tend to exclude the elderly and the unemployed.

Congress Funds Hospital Growth and Equality- By the end of WWII, congress realizes that their is a big gap between the price of heath care and what people can afford. They financed a lot of new hospitals. They also made mandatory to provide charity care. They also forbade discrimination based upon race, religion, etc., though it was still ok to provide “separate but equal” care.

The 1950′s – Labor starts adding health benefits to their collective bargaining agreements. Group health plans become more popular. In 1954, these health benefits become tax exempt.

The 1960′s and JFK – Kennedy worked very hard to develop the US Medicare and Medicaid programs. Medicare, of course, is our national health plan to cover senior citizens and disabled people. Guess what the American Medical Association called it – Socialist! Medicaid is the state and federal program to cover very poor people.

The 1960′s made some progress. A lot more people were covered. But then health care spending started to spike by the end of the decade.

1971 – Republicans and Democrats -Nixon proposed a bill that would require employers to provide a minimum amount of coverage. I mention this so you don’t only think that Democrats wanted health care reform. When Nixon was president, he also helped fund the growth of the HMO (Health Maintenance Organization) system in the US. Senator Edward (Teddy) Kennedy proposed a single payer, universal health care system for everybody. This was the start of his life-long battle to introduce comprehensive health care reform.

In 1976, Carter brought up national health care again. He did get elected, but then the nation fell into a deep recession.

The 1980′s

COBRA is signed into law in 1986. This act forces some employers to extend group health benefits to terminated employes for several months. It also requires emergency rooms to screen and stabilize all patients, regardless of health insurance or financial status.

In 1988, the Medicare Catastrophic Coverage Act prevents seniors from falling into financial ruin because of health costs. It sets ceilings on charges and the amount that Medicare beneficiaries must pay. This plan passed by a large majority, but ironically, it is repealed a few years later under pressure from advocacy groups.

President Clinton and The 1990′s

President and Hillary Clinton championed a massive overhaul of the US health insurance system. They proposed universal coverage based upon the idea of “managed competition”. i.e. health insurance would still come from private companies, but those companies would be tightly regulated. Powerful interest groups line up on the other side to defeat this idea.

The Clinton’s Health Security Act,of course, failed to pass. Why? The main reasons that experts cite are powerful lobbying and partisan politics. Does this found familiar? At that time, though, medical groups, health insurance and drug companies, and possibly the administration’s combative strategy, were thought to bring about the downfall.

CHIPS – In 1997, the Children’s Health Insurance Act did pass. This program helps cover children of low to moderate income families who lack private insurance. By 2008, over 7 million children were covered by this state and federal national health care program.

The 200′s

In the 1990′s, increased managed care had held down health care costs. But by the early 2000′s, costs had started to rise again. The Clinton plan may have been mostly buried, but the problems had only been delayed.

Bush and Medicare Prescriptions – In 2003, Bush (a Republican) expanded Medicare to cover prescription drugs. Medicare Part D was born.

Meanwhile, health care spending continues to spike. Now business groups are starting to get more concerned about the growing cost of covering employees. Some surprising companies, like Wal-mart, are beginning to suggest they would be open to the idea of health care reform. Medical groups are no longer shouting, Socialist, in the streets either. Even health insurance companies are beginning to suggest they would be open to reform if they get to participate in the process. The health insurance companies are less than excited about any more national health plans though.

Obama campaigned on a platform of health care reform. I still believe he was sincere about this, but just had no idea how tough it would be. Hillary Clinton, a veteran of the health care reform wars from the 1990′s, was more conservative in her promises. Voters didn’t think she believed in change, and Obama won. As time passed, we recognized that her more conservative statements were probably more correct.

2009 – 2010 – Well the Congress passed a bill, and then another bill got mired down in the senate. In September, the Baccus bill was proposed which is supposed to merge the two bills and add Republican ideas so they don’t get left out. Meanwhile, we are seeing quite a storm of partisan politics as everybody wants some face time.

What’s Different Now in March of 2010?

Well I don’t hear medical groups, or even private health insurance or drug companies, using the S word. Plenty of Republicans and those nutty guys on Fox news did not get the memo, so they are still using the S word. Whoops, what happened to my unbiased outline of the history of health reform?

Well, stay tuned…..

Can You Afford Prescriptios and Other Health Care?

We have published some articles on health insurance, and even ranted a bit about health reform in the US. I do not care what your political outlook is right now, but you have to know that the US health system has some big problems that affect patients, doctors, and employers. In fact, every player in this issue has a crisis to deal with right now, and I wish our own government could somehow get past the silly stuff and get some work done.

Who Needs Help Affording Health Care?

We have gotten some very sincere emails from our previous posts. I am not going to pubish them because of privacy issues. You will have to take my word for it. People are having real trouble because they cannot afford prescriptions or other health services. I think this problem is really tough for those of us betwee about 50 and 64. We are too young for Medicare, and too old to easilly qualify for private individual policies. Even if some of us have health isurance, our policies may not be all that good. I have heard of tragic cases of people running out of benefits or having huge copays, deductibles, or limits when they needed expensive medical treatment.

Solutions to Affording Prescriptions, Doctors, Clinics, Hospitals, ETC.

I am going to link to this article on finding health with health care for people who cannot afford prescriptions, doctors, hospitals, and other costs associated with being sick or injured. I think it provides a clear and succint roadmap

What to Do if You Cannot Afford Health Insurance << Click Here

The Partnership for Prescription Assistance (And Other Medical Services)

One of the big resources it mentions is PPARX.org. Though the name of the organization is the Partnership for Prescription Assistance, it does even more than provide prescription help.

This website, and organization provides a portal to private and public assistance for prescriptions, clinics, financial aids, and patient advocates. You can look up your prescriptions. You can enter a zip code and find nearby low cost (or free) clinics. You can find orgaizations that provide some financial aid for your costs or that will advocate for your treatment.

Are You Healthy Now?

This may really surprise you, but many people do not bother to get health insurance when they are still healthy because they just do not see the need. Or they may buy a policy that is very cheap, but has some limits that really make it a poor choice is a serious illess or injury crops up. I am hardly an advocate for insurance companies, but you should understand that the best time to buy health insurance is when you are healthy! You are not going to get a private insurer to cover you after you are already sick. Insurance doesn’t work like that! That would be like trying to buy life insurance after you are already dead!

Do You Have A Decent Income, But Still Lack Health Insurance?

A major medical plan with a high deductible, for a healthy person, should not cost more than the price of a couple of family restaurant meals every week. You really need to examine priorities. Besides, for many of us, health insurance premiums are tax deductible. And even if you are still responsible for deductibles and copays, you will be able to avoid financial run.

Besides, people with insurance tend to pay less for services. My own plan does not cover testing under the deductible. But when I do get those tests they are discounted. Last time I got routie blood tests, I paid about $70 for $300 of charges because of the discount. My friend, who lacks insurance, paid the whole amount. Is it fair to offer discouts to people with insurance? It isn’t fair, but it is real.

Be wary of limited benefit plans that cap out limits on the top end. An example might be a policy that pays out some figure, like $2,000 a day, for hospital stays. This is not going to be that helpful for you. You need to absorb the bottom costs, like a deductible or office visits, and have a major medical plan at your back when you really need it.

If You Cannot Afford Health Insurance or Health Costs

If you just cannot afford it, then please take time to look at the article above or the PPARX website so you can have a game plan of what to do get sick. There are organizations out there that provide prescription assistance, low cost clinics, sliding scales for hospital costs, patient advocates, etc. It would be a good idea to be aware of these before you experience a crisis.

Republicans Setting Filibuster Record In the US Senate

President Obama had an ambitious agenda when he took office. A majority of Americans voted for that agenda. However, there are rules in place that give the minority some power, and one of those is the ultimate tool of obstruction – the filibuster. Right now, Republicans are using this tool at a record place. This means that a simple majority will not be enough to get legislation through Congress.

What it also means, in my opinion, is that we taxpayers are spending a lot of money to get nothing done. It is amazing that the wheels of our nation are moving at all with these obstructionist tactics. And what is it all for?

Well the concept arose from our national sense that the majority rules, but the minority must be protected. However, in this case, I hardly think it is used to protect some disadvantaged minority, but simply for political gain. The minority, in this case the Republicans in Congress, want to score political points by keeping the majority from getting anything done. They want to stall progress, and so they want to stop progressives.

Health Reform

If you are over 50, you should be concerned about health reform. Even if you have an employer plan, you know that costs are rising, and your own coverage is at risk. A lot of large companies are asking their employees to shoulder a greater part of this burden, or they are cutting benefits. If you are a small business owner, you know how much more expensive it is to keep a good health care plan for your family and employees. This problem is hurting business! Many companies, large and small, are in favor of US health reform.

The only group that I can see to benefit from a lack of progress are the insurance companies. We need to look at our problems and will make sensible changes that will benefit health consumers (all of us) and medical providers.

The health insurance companies, in my opinion, should be at the bottom of our concerns. Why aren’t they? That is a question you should ask. One other question you should ask is how our representatives would be reacting if they had to purchase their own health insurance or depend upon Medicare.

Having Trouble Keeping up with Health Reform?

If you are having a hard time keeping the health reform proposals straight, I have to tell you that you are not alone! Wow, I believe this is what we call a total SNAFU. The Obama White House, such strong advocates of health reform when running for office, has been strangely silent. As of this morning, as I write this, Howard Dean said the whole thing should just be scrapped for a do-over. I have to admit that this same thought has crossed my mind quite a few times in the past few weeks.

Yes, I am a Liberal

What do you want out of me? I grew up in Mitchell, South Dakota, the land of prarie politics and George McGovern. We were clear eyed idealists back when I was a Young Democrat who made phone calls for that campaign. I wasn’t old enough to vote then, but now I have been old enough to vote for a long time. The more things change….. Well, you know what it means now.

I also Worked in the Insurance Industry

I am not the only person who worked in insurance who supports health reform, but I am probably in a minority. I saw the problems with finding private coverage for people with health problems, and also saw how hard it was for middle class families to afford premiums. These are working people we are talking about, and not the very poor. Very poor people can probably fall back on Medicaid, and that is not a middle class option.

What is Next For Health Reform in 2009 or 2010?

Well, I am sure you have noticed that 2009 is almost over, so we are probably going to see this painful process continue into next year.

Right now, the curent bill seems to support that industry more than it does the community. It forces everybody to have health insurance, but provides no public option as a fallback. We saw the rise of the Medicare at age 55 idea, and were a little bit optimistic that could lead to further expansion of this established health system. But that idea seems to have been knocked down too now.

What Will Cover the Uninsured?

I wish I could give you a clear summary of how this current bill will help millions of uninsured people, but I have no clear idea. If you fine people for a lack of coverage, how do you help them get it? <shrugs> All we seem to read on the news are stories about plans that have been tabled, but no real stories about reforms that will be working.

Who Favors the Public Option?

According to polls, a majority of Americans do favor a public option. Even many businesses are on board because they are struggling with spiking health care costs. The idea of having a business fund their own group plan or contributing to funding of a public health plan seemed to have some merit, but it seems to have vaporized in the fog of misinformation and politics.

What if You Need Health Insurance?

If you need a medical plan, start here – No Medical Insurance. You can also do some online shopping with competitive individual health insurance quotes. You can find premiums and plans from local companies who want to compete for your business. If you have had trouble getting accepted in the past, try finding a health insurance agent who can help you find options. A qualified options should be familiar with local private plans, and also know something about existing government options.

Try this – Free Insurance Agent Directory

Is Health Reform Still Alove and Kicking?

I think that most of us who pulled with YES switch on Obama, or at least the Democratic party, were attracted by promises of finally getting a health reform bill passed. When Hillary Clinton said it might not be so easy, we did listen. And now she is probably having a great time saying, I told you so! But the health reform bill still seems to be slogging through Congress, though not exactly the way we thought it would be!

Some Americans Health Care Reform Could Help

Do you think you know who health reform will help? You may be overlooking some groups if you think it will only help a bunch of bums who plot for a socialistic takeover. Sorry, that was a bit harsh, but I guess I caught too much Fox News yesterday.

Women Do you know that, while life insurance premiums are usually lower, health insurance premiums will be higher for a man than a woman – other factors being equal. While women tend to live longer, we also need (or at least seek) more medical care. Food for thought, there…..

People with Health Conditions Do you realize that private insurers can decline applicants for coverage based on some fairly minor health conditions. Dou you know that, in some US states, it is even legal for insurers to decline former victims of domestic violence? This has got to stop!

Small Business Unemployed, self-employed, or contract workers are not the only people who have issues with insurance premiums. Many small businesses are struggling with rising premiums because their group is too small to really spread the risk well. And while individual insurance plans usually cannot raise rates because an insured person got sick, a small group policy can do it!

Picture a typical situation. A small machine shop has 8 employees. One worker has a heart attack and has a small group plan cover most of his expenses. A few months later gets a notice that the group premium is going to double in 30 days. Does this happen? It happens all the time!

People over 50 People between 50 and 64 are pretty tough to cover in the US if they have to seek an invidual policy! Middle aged people are bound to have more health conditions than they did 10 or 20 years ago, and high blood pressure, etc. can get a person rated up or declined today!

These are just some of the groups who are having a tough time with the current system. We’d be pleased to see your comments on the issue of Obama’s health reform efforts. Just register and post!

Medicare vs Health Reform?

If there is one US institution that is sacred with US seniors, it is Medicare. Medicare is the public health insurance option for American senior citizens and disabled people. If you are not familiar with Medicare, you can get a quick primer here: Compare Medicare Health Insurance.

Is Medicare Health Insurance Radical?

But it may surprise you to know that the beginning of this public health option were very hotly debated across all segments of society in the early 1960′s. And now, 50 years later, it is just part of the fabric of America. I wonder if some of they pundits on Fox News have gone back and look at some newspaper articles from the very late 1950′s or early 1960′s.

Health Insurance Reform Myths

I am not that big of a fan of AARP, because I find them a big marketing wing for insurance companies rather than the special interest group for the over 50 set they claim to be. But to my pleasant surprise, they have been very supportive of the idea that health care change has got to come! And why not? Even Wal-mart and many other large businesses support government reform.

It did not take a lot of Googling to find an AARP article on health reform myths and facts. Look at what AARP has to say.

Fact: None of the health care reform proposals being considered by Congress would cut Medicare benefits or increase your out-of-pocket costs for Medicare services.

Fact: Health care reform will lower prescription drug costs for people in the Medicare Part D coverage gap or “doughnut hole” so they can get better afford the drugs they need.

Fact: Health care reform will protect seniors’ access to their doctors and reduce the cost of preventive services so patients stay healthier.

Fact: Health care reform will reduce costly, preventable hospital readmissions, saving patients and Medicare money.

Fact: Rather than weaken Medicare, health care reform will strengthen the financial status of the Medicare program.

Bottom Line: For people in Medicare, health care reform is about lowering prescription drug costs for people in the “doughnut hole”, keeping the doctor of your choice, improving the quality of care, and eliminating billions in waste that is causing poor care and medical errors.

The aim of health care reform is not to hurt seniors, but to help everybody. Seniors are important, but so are middle aged people, young adults, and children. We need to move together if we want to move ahead.

Personally, I believe it would be better to move ahead and stages. For intance, maybe we should address the issues of uninsurables, and then see how many uninsured people are left. We could attack a problem at a time. But that is a moderate stance, and apparently it is getting drowned out in the firefight between the convervatives and liberals.

But don’t get me wrong. I do believe it is time for health care reform, and as a self-employed person, I struggle with rising premiums and costs like many others do.

Money is The Only Thing

But as a business person, I do understand that the big issue here is money. This is a tough problem when many (most?) families generate more in health insurance premiums than they pay in taxes!

As a very smart person I know said, “The only real issue here is money.”
